MAINTAINERS: Remove Eduardo Habkost
Eduardo Habkost doesn't wish to be listed in MAINTAINERS any more; remove his email, and also the git trees he used to use. Move the QOM boilerplate conversion script scripts/codeconverter to "Orphan" status, as it no longer has any listed maintainers.
