Merge feature/streaming-ux: GUI-launch PATH fix and streaming tool calls

Fixes `spawn sigit ENOENT` when VS Code is launched from the GUI, and adds in-place streaming of tool calls with a progress bar. Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>
